The Mesa, AZ Holistic & Naturals Medicine location is in Pierpont Plaza at the southwest corner of Pierpont Drive and Banner Gateway Drive.

The Scottsdale Holistic & Functional Medicine location is south of the southwest corner of Bell Road and Thompson Peak Parkway, inside of AZ Multicare.
